Mentre le guardie fermano Manche, gli altri otto dopo aver scavalcato il muro di cinta si nascondono nei pressi del Fox River in attesa di muoversi. Per prima cosa, si liberano di Haywire e salgono su un furgone lasciato da un uomo di Abruzzi per raggiungere il piccolo aeroporto. La fuga non si rivela però affatto facile: i fuggiaschi sono infatti circondati dalle guardie in tutte le direzioni e ben presto sono costretti a lasciare il furgone. T-Bag, per evitare che venga ucciso, si ammanetta a Michael; più tardi però Abruzzi decide di utilizzare il metodo più veloce per lasciarlo dietro, amputandogli il polso sinistro. Veronica giunge a Blackfoot e si presenta di fronte a Terrence Steadman. Al Fox River, intanto, si comincia a indagare sulla fuga e Pope capisce che Sarah li ha aiutati lasciando la porta dell'infermeria aperta. Una pattuglia si reca nell'appartamento della dottoressa ma la trova in overdose di morfina, apparentemente morta. La Vicepresidente capovolge la situazione che la vedeva ormai esautorata e dopo aver provocato la morte del Presidente, assume la Presidenza degli Stati Uniti. A causa di una soffiata, la polizia si reca nell'aeroporto e l'aereo di Abruzzi si allontana poco prima l'arrivo dei ragazzi: ormai a Michael, Lincoln, Sucre, Abruzzi e C-Note non rimane che correre nei boschi che circondano la prigione.

Note & Curiosità
Quando l'epiosodio è stato trasmesso in Inghilterra c'è stata una narrazione iniziale e di chiusura da parte di Wentworth Miller. Questo il testo di quella iniziale: "I'm Michael Scofield, I wanted to be here for the end of my story. It all began in January and now we've reached the final chapter of Five's Prison Break. As you know by now Fox River isn't a friendly place, so you're going to see some violence"; mentre quello di quella conclusiva: "And the second season of Prison Break will be back on Five next year".

T-Bag: Think twice, dumb boy. You shoot me, pretty here will be draggin' around 175 pounds of dead Alabama flesh with him. And considerin' how much you need him to get this little Fibonacci vendetta of yours, huh? So I just don't think you're gonna be pullin' that trigger.

Michael: This is as far as you go, my friend.
Tweener: Wha? No man, you can't leave me out here, not like this...
Michael: We had a deal, remember?
Tweener: Yeah, but come on bro…
Michael: You and I, we're not bros. And I don't think you want me tellin' the guys what I know. Now walk.

TV Reporter: …have corroborated that President Mills was admitted into emergency care here a little more than 45 minutes ago. In the chief surgeon's words, President Mills suffered massive cardiac arrest. Doctors were unable to revive him. The news out of Washington Medical Center is official now – the president of the United States, Richard Mills, is dead. And keeping with protocol Vice President Reynolds is in a undisclosed location being sworn in as the 46th president.

Pope: You're saying you don't know anything about this?
Katie: Honestly, I don't.
Pope: The door was left open, Katie.
Katie: It wasn't me!
Pope: Then who was it? Only you and Dr. Tancredi were left at the end of the night. The orderly confirmed that.
Katie: It wasn't me!
Pope: Then you're telling me it was Dr. Tancredi?
Katie: No! I…
Pope: I know you know something,
Katie. And anything you know, you have got to tell me. If you don't you are gonna lose a lot more than your job. Withholding information makes you an accessory and I will press that button if you don't start talking!
Katie: Sara…
Pope: What about Sara?
Katie: Sara had a thing for Scofield.

Pope: When those men went over the wall they made a choice. That choice makes them a threat to society once again. Now as many of you know, some of those men are convicted killers. Now our job is to protect and ensure the safety of the general population. That means, if we have to bring them down to ensure that safety, by God we will do it! This time we're playing for keeps, gentlemen. Now it's been at least 12 minutes since they went over that wall. That's 1 mile on foot at best. If they've got their hands on a vehicle they could be 10 miles from here. So LETS GET MOVING!

Bellick: Get me my shotgun!
Random Prison Guard: Pope's already given orders for all the C.O.s to hit the...
Bellick: Did I stutter?! Get me my shotgun! Those piles of crap doing what they just did, just signed their death warrants. Every last one of them.

Abruzzi: If you think I won't cut you myself to get that key, you're in for a big surprise.
T-Bag: I encourage you to remember the last time you came at me with a knife...
Michael: Shut up! Shut up! Both of you!

T-Bag: Hey, John, I gotta ask you, Why are you so intent on Lincoln driving here and you taking that particular seat.

Haywire: Sorry.

Abruzzi: He's lucky I didn't take this to his head.

T-Bag: I guess it was just meant to be.
