Bellick viene colpito da Westmoreland e nascosto nel buco. L'uomo avverte Michael che bisogna immediatamente evadere. Michael è costretto a coinvolgere Sara nella fuga: le dice di lasciare semplicemente la porta dell'infermeria aperta. Veronica viene fermata da Nick che le rivela di lavorare per Abruzzi in cambio della libertà di suo padre. Sara viene a sapere che il padre è legato alla Vicepresidente Reynolds e che la donna gli ha promesso la carica di vice nel caso venga eletta alle prossime elezioni: un duro scontro con lui la porta a riflettere a lungo. Con Lincoln chiuso in isolamento, Michael ha un'unica possibilità per portare il fratello con lui: minacciare il Direttore Henry Pope.
